Refund Policy

At Nuform Health, we are committed to delivering a high-quality, physician-led experience. Please review our refund policy carefully, as it differs by service type.

Annual Physician Fee

  • The physician fee is refundable up until your consultation begins.
  • For hormone therapy patients, the fee remains refundable until blood work is completed.
  • If your provider determines that you are not a candidate for treatment, the physician fee is refunded in full.
  • Once treatment has been recommended and accepted, the physician fee is non-refundable.

Consultation Fee ($50)

  • A $50 consultation fee is collected at checkout to reserve your appointment with a Nuform provider.
  • The fee is refundable in full if your provider determines you are not a candidate for treatment.
  • The fee is refundable in full if you cancel your appointment at least 24 hours before its scheduled time.
  • Once your consultation is complete, the fee is non-refundable.

Medications

  • All medication orders are final at the time of purchase.
  • Once a medication has been ordered with the pharmacy, we are unable to provide a refund.
  • Medications cannot be returned to the pharmacy or clinic once dispensed.

Blood Work & Diagnostic Labs

  • Once blood work has been completed, we are unable to issue a refund.
  • Blood work is refundable up until the time of completion.
